Most herniated disks occur in the lower back, but they also can occur in the neck, rarely in the thoracic region (upper back)

What is Disc Prolapse?
Disc prolapse, also known as a herniated or slipped disc, occurs when the soft, jelly-like material (nucleus) in the centre of a disc pushes through a tear in the tougher exterior covering (annulus)
This condition can cause pain, numbness, or weakness in an arm or leg if the bulging disc material compresses nearby nerves.
What are the Causes of Disc Prolapse?
Disc prolapse can be caused by several factors, including:
- Aging: As we age, disc lose some of water content as part of degenerative process leading to weakening of the disc and more susceptible to tears
- Injury: Sudden trauma or injury can cause a disc to herniate(can force a disc out of confined place)
- Repetitive Strain: Repeated heavy lifting or twisting motions can strain the spine, leading to disc prolapse.
- Obesity: Excess body weight puts additional stress on the discs in the lower back.
What is the Anatomy of Disc Prolapse in the Spine?
Each spinal segment is made up of vertebrae and intervertebral discs. These discs act as cushions between the vertebrae, absorbing shock and allowing flexibility in the spine.
Each disc has a tough outer fibrous layer (annulus fibrosus) and a soft, gel-like centre (nucleus pulposus).
When the annulus tears or weakens, the nucleus can bulge or herniate, potentially impinging nearby nerve roots.

What are the Common Symptoms Associated with Disc Prolapse?
Common symptoms associated with disc prolapse include:
- Localized Pain: Pain in the affected area of the spine, often in the lower back or neck.
- Radiating Pain: Pain that spreads to the arms or legs, often due to nerve compression.
- Numbness or Tingling: Sensations of numbness or tingling in the affected extremities.
- Muscle Weakness: Weakness in the muscles served by the affected nerves(Foot drop , difficultly in grip foot wear in the toes, difficulty in griping daily objects in hands)
- Loss of urinary control – urgency , trouble urinating even with a full bladder.
If I Have Disc Prolapse, Should I See a Spine Doctor Always?
You should see a spine surgeon , if you have the following
Localised pain – The pain is severe or does not improve with rest(Neck and back)
- Pain that radiates into your arms or legs(Sciatica)
- You have numbness, tingling, or weakness in your extremities(hands and legs)
- The pain is associated with a recent injury or trauma.
- You experience loss of bladder or bowel control(inability to control, urgency), saddle anaesthesia (numbness around the buttocks/genitals).
- Symptoms lasting more than 4–6 weeks despite conservative care.
Can I Treat My Disc Prolapse with Household Remedies?
Mild cases of disc prolapse can sometimes be managed at home with:
- Rest: Avoid activities that triggers the pain, avoid prolonged best rest
- Ice/Heat Therapy: Ice for 20 minutes early on, then heat to relax muscles.
- Over-the-Counter Pain Relievers: Medications like ibuprofen or acetaminophen with muscle relaxants can help alleviate pain.
- Gentle Exercise: Stretching and strengthening exercises may help improve symptoms – Pelvic tilts, knee-to-chest for lumbar, neck stretches for cervical.
- Proper Posture: Maintaining neutral posture to reduce stress on the spine.
What are the Treatment Options for Disc Prolapse?
Treatment options for disc prolapse include:
- Medications: Anti-inflammatory drugs , muscle relaxants and neuropathic pain medications
- Physical Therapy: Exercises to strengthen the back muscles and improve posture.
- Epidural Steroid Injections: To reduce inflammation around the affected nerves
- Surgery: In severe cases, surgery may be necessary to remove the herniated disc material to relieve compression on the nerve
Preventive Strategies
- Core Strengthening: Supervised physiotherapy, or targeted gym routines.
- Weight Management: Maintain a healthy BMI to reduce stress on spine
- Ergonomics: Adjustable chairs, lumbar rolls, and sit-stand desks.
- Safe Lifting Techniques: Hinge at hips, keep objects close to your body.
- Regular Movement: Break up prolonged sitting every 30–45 minutes.
Quit smoking
- Over-the-Counter Pain Relievers: Ibuprofen, acetaminophen, naproxen.
- Muscle Relaxants: For short-term relief of muscle spasms.
- Prescription Pain Medications: For more severe pain, under a doctor's guidance.
- Corticosteroids: To reduce inflammation and swelling.
- Non-surgical treatments have not provided relief after several weeks or months.
- There is significant nerve compression leading to weakness, numbness, or pain.
- There is a need to remove the herniated portion of the disc or stabilize the spine.
